« De l’autonomie d’un acteur non-institutionnel, Réunisaf, à la logique de la coordination des acteurs de santé : les paradoxes de la participation et de l’accompagnement des femmes enceintes en risque d’alcoolisation fœtale à La Réunion ».: La participation dans les politiques sociales et médico-sociales et ses limites : l'exemple de l'accompagnement autour du Syndrome d'Alcoolisation Foetale.

Abstract : The translation of a problem of congenital intoxication into a medico-social, psycho-social problem, into "health risk" and then into a public health priority has been, in Reunion, the fact of REUNISAF, an actor associative and this, from the 2000s ... This one tends to make up for the failures of the State in the prevention of the Disorders caused by Fetal Alcoholization. Participation-innovation of REUNISAF goes through a cognitive mixing with the regulatory norms and relies on an psychosocial referent in the treatment of the "pregnancy and alcohol" problem. The alliance of the associative actor and the senator of Reunion, Anne-Marie Payet, extends the participation to the construction of a priority of health around FAS and FASD enshrined in the law in 2005. But the new administrative culture of 2010 around health networks will build a new cognitive framework to say differently risk. The protocolisation of prevention and addiction therefore takes the form of a classic implicit remedicalization of female addiction. In addition, the depreciation of the correlated participatory model leads to a depoliticization of risk and thus to a weakening of support for health autonomy previously invested by REUNISAF. A new taxonomy of women at risk of fetal alcohol abuse is emerging which contributes to increased empowerment of women without taking full account of their subjective fragility. It is the autonomy of action of a prevention actor, the risks of participation in medico-social public policies, the depreciation of the participatory model, the emergence of a new cognitive and normative framework and their consequences for accompaniment. women at risk of fetal alcohol, questioned during the first half of 2017 in Reunion, which we propose to present here.
