L'invention du contrôle médico-sportif en France. Entre protection de la jeunesse et contrôle des fédérations sportives, retour sur le développement d'une "médecine préventive"

Abstract : In 2005, when french deputy Olivier Jarde pleaded in favor of the "necessary revaluation" of a sports medicine that he considered little recognized academically, are invoked in concert the "exponential growth of health expenditure" and " 26 million practicing sportsmen ". With such arguments sports medicine is and must be a preventive medicine. If branches of medicine have taken possession of the sporting object to the point of circumscribing sets of knowledge and practices sufficiently specialized to be specifically designated -sport traumatology, sports cardiology- the fact remains that sport medicine is immediately associated with the medical examination that leads (or not) to the signing of the certificate of non-contra-indication to the practice of sport. This examination assigns physicians to a medical work on healthy man and condemns this medicine to occupy a subordinate position in the hierarchy of medical specialties. Olivier Jarde's parliamentary report speaks of "discipline not found", to qualify a control medicine, which is imposed on a crowd of sportsmen. In this chapter, we will review the conditions that led to the construction of such an exercise in the middle of the 20th century. For the benefit of political work of administrative formatting of an exercise entirely dedicated to control, screening and hygiene, public funding, made legitimate by the desire to supervise the youth and control the federations, ensure indeed, this sports medicine of the 1950s brought a whole new audience to the national territory and made it a "community medicine". This is the paradox; sport medicine finally obtains from the state the means that it has been demanding for 30 years and becomes visible, but the focus on its preventive dimension condemns it to occupy a position dominated in the order of professional prestige at the very moment when the French medical field is part of a process of specialization.
