Perceived vocal straining for 84 dysarthric patients: free verbalizations, perceptive quantitative assessment and agreement between four expert listeners

Résumé

Four expert listeners, all speech therapists and phoneticians, conducted a perceptual evaluation of the overall grade of dysarthria, the GRBAS, and they freely verbalized the voice and the speech of 84 dysarthric patients (39 patients with Amyotrophic Lateral Sclerosis ALS, 23 patients with cerebellar syndrome and 22 patients with Parkinson's disease). The corpus is a read three minutes long text "Le Cordonnier" (Fougeron et al. 2010). Analysis of free verbalizations was carried through the counting and semantic categorization of obtained words. Our results show that the overall grade of dysphonia GDysph is perceived with a similar degree between the listeners, and it isn't significantly different from the overall grade of dysarthria GDysar. The perceived vocal straining elements the most severely assessed are roughness (1.1, SD 0.8) and the strained voice (10.9) for ALS patients, the breathiness for Parkinson patients (1, SD 0.8) and the strained voice for cerebellar patients (0.8, SD 0.8) with a great variability in the obtained data. The importance of judging these patients as dysphonic ones is thus justified since the inter-listeners agreement within ± one scale value is situated between 95 and 100% for the GDysph and the GDysar of ALS patients, the GDysph, the breathiness and laryngeal asthenia for cerebellar patients, and the roughness and the GDysph for Parkinson patients. About 2000 occurrences of free verbalizations were written by the four expert listeners, with an average of 5.9 verbal units per listener per patient (SD 3). Among the seven semantic classes of these verbal units, results for the three pathologies show a similar distribution of percentages of occurrences across the four listeners: half of the total concerns the speech, and a quarter concerns the voice (a third for ALS patients).
Quatre auditeurs experts, tous orthophonistes et phonéticiens, ont effectué une analyse perceptive du grade global de dysarthrie, du GRBAS, et ont aussi librement verbalisé la voix et la parole de 84 patients dysarthriques (39 patients avec une Sclérose Latérale Amyotrophique SLA, 23 patients avec un syndrome cérébelleux, et 22 patients atteints de maladie de Parkinson). Le corpus est un texte lu de trois minutes " le Cordonnier (Fougeron et al. 2010). L'analyse des verbalisations libres a été effectuée par l'intermédiaire du comptage et de la catégorisation sémantique des mots obtenus. Nos résultats montrent que le grade global de dysphonie GDysph est perçu avec un degré similaire parmi les auditeurs, et n'est pas significativement différent du grade global de dysarthrie GDysar. En outre, les éléments de forçage vocal les plus sévèrement cotés sont la raucité (1.1, ET 0.8) et la voix serrée (10.9) pour les patients SLA, le souffle pour les patients Parkinson (1, ET 0.8), et la voix serrée pour les patients atteints de syndrome cérébelleux (0.8, ET 0.8) avec une grande variabilité dans les données obtenues. L'importance du jugement de ces patients comme dysphoniques et aussi justifiée par le fait que l'accord inter-auditeur à un point près est situé entre 95 et 100% pour le GDyph et le GDysar des patients SLA, le GDysph, le souffle et l'asthénie laryngée pour les patients cérébelleux, et la raucité et le GDysph pour les patients Parkinson. Environ 2000 occurrences des verbalisations libres ont été écrites par les quatre auditeurs experts; avec une moyenne de 5.9 unités verbales par auditeur et par patient (ET 3). Parmi les sept classes sémantiques obtenues pour ces unités verbales, les résultats pour les trois pathologies ont montré une distribution similaire des pourcentages d'occurrences parmi les quatre auditeurs: la moitié du total concerne la parole, et le quart concerne la voix (un tiers pour les patients SLA).
