Résumé

Health is not limited to the sole medical sector. The WHO defines it as " a state of complete physical, mental and social well-being". Thus, the hospital or the doctor cannot answer alone to all health needs. A whole body of places, services, listening- and meeting groups exists in the medical and social fields, and are less institutionalized than within the strict framework of the medical part of healthcare. In this nebula, the place of the individual is broad and, in France, a whole portion of proximity services is offered by non-profit organizations (associations act, 1901), which sometimes rest on voluntary help and on militant implication; sometimes on more complex organizations, quasi institutionalized. The role of associations of health in the local organization of the social and medical networks is a subject still little studied in spite of some rich researches on the associative fact. What is the place of those associations in the local systems and what role do they play? This text brings some answers to this question on the basis of a double investigation (qualitative and quantitative) led in the West of France about the associations of the medical and social sectors.
La santé ne se limite pas au seul secteur médical. L'OMS l'a définie comme "un état de bien être complet, physique, moral et social". L'hôpital ou le médecin ne peuvent donc pas à eux seuls répondre à la totalité des besoins de santé. Tout un ensemble de services, de lieux d'accueil, d'écoute, de rencontre, d'échanges existe dans le domaine sanitaire et social et de façon beaucoup moins institutionnalisée que dans le strict cadre du volet médical de la santé. Dans cette nébuleuse, la place de l'individu est plus large et, en France, tout un pan de l'offre de services de proximité est le fait d'associations à but non lucratif (loi 1901) qui reposent tantôt sur une large part de bénévolat et d'implication militante des adhérent tantôt sur des organisations plus complexes, quasi institutionnalisées. Le rôle des associations de santé dans l'organisation locale des réseaux sociaux et sanitaires est un sujet encore peu étudié malgré quelques riches travaux sur le fait associatif.
Comment les associations se positionnent-elles dans les systèmes locaux et pour y jouer quel rôle ?
Ce texte apporte des éléments de réponses à cette question sur la base d'une double enquête qualitative et quantitative conduite dans l'Ouest de la France auprès des associations du secteur sanitaire et social.
